Why this grade

Several published, placebo-controlled human trials, all in The Lancet, but no Phase 3 results yet. A first-in-human Phase 1 trial (oral, 144 people) and a Phase 1b/2a trial (injected, 125 people) showed large weight loss, up to about 24% at 36 weeks at the top dose, but they were small, single-site, and the injected study had many dropouts. Two 36-week Phase 2 trials in type 2 diabetes (2026) showed clear blood-sugar improvements, with up to 14.6% weight loss reported by the company for the injection. Novo Nordisk started Phase 3 trials (AMAZE for obesity, AMBITION for diabetes) in 2026; none has reported. All trials were sponsor-funded and no regulator has approved it.

What is it?

Why people buy it

It is being tested for weight loss and type 2 diabetes. It may come as a weekly injection or a daily pill.

What we actually know

  • •It is one medicine that copies two fullness hormones at once. One is the hormone that Wegovy copies. The other is amylin.
  • •In a small study, people on the highest dose lost about 24% of their weight in 36 weeks.
  • •Many people dropped out of that study. The big studies have not finished yet.
  • •It is not a licensed medicine in the UK or anywhere else. Its new official name is zenagamtide.

What might happen

  • •Big weight loss is possible, going by early studies. This is not yet proven in large studies.
  • •Feeling sick, being sick and other stomach problems were very common in the studies.
  • •Long-term safety is unknown.
  • •Anything sold online with this name is not the tested product. Nobody checks what is in the bottle.
  • •It is banned in sport at all times, because it is not an approved medicine.

Bottom line

Very strong early results, but from small studies. Wait for the large studies and a real licence.

How is it meant to work?

A single peptide that activates both the GLP-1 receptor and amylin receptors (and calcitonin receptors). GLP-1 activity boosts glucose-dependent insulin release, suppresses glucagon and reduces appetite; amylin activity adds fullness signalling through a separate brain appetite system and slows gastric emptying. Combining both in one molecule aims for greater weight loss than either alone. Given as a once-weekly subcutaneous injection or a once-daily oral tablet in trials.

Does the human evidence stack up?

Promising, published, but early. A first-in-human Phase 1 trial of the tablet (Lancet 2025, n=144) found it tolerable, with mostly gastrointestinal side effects. A Phase 1b/2a trial of the injection (Lancet 2025, n=125, single US site) reported about 24.3% mean weight loss at 36 weeks at the top dose versus 1.1% on placebo, but many participants withdrew. Two 36-week Phase 2 trials in type 2 diabetes (Lancet 2026; 262 on injection, 186 on tablet) showed significant HbA1c reductions versus placebo; Novo reported up to 14.6% weight loss with the injection in a company release. All trials are sponsored by Novo Nordisk. Large Phase 3 trials began in 2026 and have not reported, and there are no long-term safety data.

What could go wrong?

  • !Not licensed by the MHRA or any other regulator. It is investigational and its benefit-risk balance has not been signed off
  • !Gastrointestinal side effects (nausea, vomiting and similar) were very common in early trials and rose with dose
  • !The headline weight-loss figures come from a small, single-site Phase 1b/2a trial with many withdrawals; large Phase 3 results are not yet available
  • !No long-term safety data
  • !All trials are run and funded by the manufacturer
  • !The name change to zenagamtide means it may be sold or discussed under either name
  • !GREY MARKET: anything sold online as 'amycretin' or 'zenagamtide' would be an unlicensed research chemical of unverified identity, dose, purity and sterility, not the trial product

Is it legal in the UK?

Investigational drug, not a licensed UK medicine. The MHRA has not granted a marketing authorisation for amycretin (zenagamtide) as an injection or tablet, and it is not available on prescription or the NHS. Legitimate access is only through a registered clinical trial. Anything sold online under either name is an unlicensed research chemical; supplying it for human use would breach the Human Medicines Regulations 2012.
